Senior Nutrition Program

Mama’s Kitchen is proud to provide Senior Home-Delivered Meals through the County of San Diego’s Older Californian’s Nutrition Program.

PROGRAM OFFERINGS:
This program provides up to two (2) medically tailored meals daily, home-delivered to the individual’s home. Deliveries are made twice per week on Tuesdays and Fridays. Individuals must be home to receive their delivery.

ELIGIBILITY:

Individuals must meet all of the following criteria for this program:

Be 60 years of age or older
Reside in one of the following zip codes: 92111, 92117, 92121, 92126, 92103, 92104, 92105, 92115, 92116, 91941, 91942, 91977, 91978, 91910, 91911, 91913, 91914, 91915, 91950, 91952

*If you are under 60 years of age or live outside these zip codes, please see our Get Help page for additional eligibility options.

Additionally, individuals must meet at least one of the following criteria:

Have a diagnosis of cancer, HIV, type 2 diabetes, congestive heart failure, or chronic kidney disease and are unable to cook or shop for themselves.
Unable to perform at least two activities of daily living, including bathing, toileting, dressing, feeding, breathing, transferring, and mobility and associated tasks, without substantial human assistance, including verbal reminding, physical cueing, or supervision.
Have cognitive or other mental impairment requiring substantial supervision and care coordination needs.

Restrictions: Individuals receiving other congregate or home-delivered meal services are not eligible for Mama’s Kitchen’s services. Individuals may still receive CalFresh benefits and receive Home-Delivered Meals.

HOW TO ENROLL

Complete the County of San Diego Home-Delivered Meals intake form and submit it to secure@mamaskitchen.org or by fax to 619-233-6283. Once received, our team will contact you within 48 hours.

If you need additional support, please contact us by phone at 619.314.5789 for an initial assessment.

For clients qualifying for this program, an in-home visit will occur within two weeks of the initial assessment. Clients are visited quarterly to assess continued qualification for the program.
